What to prepare
- Prior operative report(s) — Full operative note, not just discharge summary.
- Pathology report(s) — Confirms whether tissue removed was endometriosis.
- MRI report and images — Ideally endometriosis-protocol MRI.
- Pelvic ultrasound report and images — Ideally with bowel/deep-disease evaluation.
- Fertility work-up — AMH, antral follicle count, prior IVF cycles.
- Medication list — Current hormonal therapy, pain management, other.
- Symptom history — Pain map, cycle pattern, bowel/bladder symptoms, timeline.
- Prior treatment list — Hormonal trials, surgeries, physical therapy, pain interventions.
How to submit
- Contact an advisor via the intake form and check "instructions to upload my medical records."
- You will receive secure upload instructions by email.
- Do not send full-resolution imaging discs by post unless requested.
- A specialist team may take 5–10 business days to complete a structured review, depending on complexity.
